Kosovo Health Information System: Building Digital Healthcare Infrastructure
How can a developing nation build a modern healthcare information system from the ground up? We helped design and implement a nationwide Health Information System for Kosovo — connecting patients, healthcare professionals, and institutions into one integrated digital ecosystem.
Challenge
Kosovo's healthcare sector lacked a unified digital infrastructure to manage patient data, coordinate care across institutions, and support evidence-based decision-making. Paper-based processes, fragmented records, and no standardized data exchange made quality healthcare delivery and national health planning extremely difficult.
Beyond technology, the challenge was building capacity within the Ministry of Health and healthcare facilities. Staff needed training, clinical and financial processes required redesign, and the entire system had to align with international standards like the HL7 Functional Model while respecting local governance structures.

Approach
We developed a comprehensive Health Information System covering four key domains: patient care, registers, security, and enterprise planning. Working with the Luxembourg Agency for Development Cooperation and Kosovo's Ministry of Health, we implemented electronic health records, patient management systems, order management, and robust identity and consent management frameworks.
The system provides healthcare professionals with a single, up-to-date source for patient information — from blood type and allergies to treatment history and lab results. It enables continuity of care, prevents medical errors through automated safety checks, and supports informed clinical decisions.
Outcome
By project completion, the HIS was implemented in 61 health facilities covering 30% of public-sector healthcare, with 4,000 end users, 350,000 registered patients, 650,000 episodes of care, and 1.1 million transactions. Training achieved 85% user satisfaction, with staff able to apply skills immediately.
The system established foundational registers for healthcare professionals, public health institutions, patients, and pharmacies — creating the data infrastructure needed for national health statistics, public health reporting, and future healthcare modernization across Kosovo.
